We hate having to turn down your requests. RTKL requests that are filed using the official and most up-to-date form (located via the links below) from the state of Pennsylvania will be accepted. (All other formats, hand-written, typed via email, or using any other form will be denied). Please be aware that your request may need both forms.
If your request is for a Police Report, please complete the Right-To-Know form.
If you haven’t already, download and fill out a copy of the PA Standard Right-To-Know Law Request Form from www.openrecords.pa.gov.
You can submit this form to us using the digital submission option below or in person. If you have not received a response within 3 business days, please contact our office during normal business hours, Monday-Friday, 8am – 4pm at 724-625-3157.
