Only RTKL and Act 22 requests that are filed using the official and most up-to-date form (located via the links below) from the state of Pennsylvania will be accepted. (All other formats, hand-written, typed via email, or using any other form will be denied). Please be aware, your request may need both forms.

Pennsylvania’s Act 22: Law Enforcement Recording applies to individuals seeking “any audio recording or video recording made by a law enforcement agency.” The full text of our state’s Act 22 can be found here. (You will be redirected to an official state document.)
